The Active Head Restraint on the front passenger side headrest failed. The headrest thrust forward at a high velocity, injuring the occupant of the seat. The car was stationary at the time of the incident.

While driving down the road the van just shut down in the middle of the road as other cars are going by honking the horn and coming close to hitting us with children in the van and no way to get off the road without the engine running. We gave contacted dodge in dallas GA they confirmed a recall for the issue but proceed to say thus VIN is not included. I pray we don't stall while trying to turn into somewhere and a car t-bone us. If we survive I will make sure to sue Dodge this is a horrible problem and very dangerous.

The contact owns a 2013 Dodge Grand Caravan. The contact stated that while driving at an undisclosed speed, the vehicle failed to accelerate as intended while depressing the accelerator pedal, and the vehicle stalled. The contact was unable to pull over to the side of the road. No warning lights were illuminated. The vehicle was left on the side of the road. The contact returned and towed the vehicle to an independent mechanic who diagnosed that the fuel pump relay inside the Totally Integrated Power Module (TIPM-7) had failed. The vehicle was not repaired due to the cost. The vehicle was towed back to the residence and had not been driven since. The contact related the failure to NHTSA Campaign Number: 15V115000 (ELECTRICAL SYSTEM); however, the VIN was not included. The manufacturer was notified of the failure and confirmed that the VIN was not included in the recall. The failure mileage was approximately 190,000.

After turning the key on, van started and then turned off. Van was restarted and drove normally for awhile. While driving. the van turned off with no notification or warning lights. This almost caused a DEADLY accident. The fault is with the ignition box. Other model years were recalled for the same reason. Dodge failed to make any changes to their known problem with the ignition or recall model year 2013. This is a known problem with the Dodge Caravan and needs to be addressed before someone is killed. I still have the part as I can not replace it without taking it to a Dodge dealership to program it. Dealership wants to charge an exorbitant amount that should be paid for by Dodge.

The check engine warning light was illuminated. Upon further inspection, the contact became aware of excessive white smoke coming from the exhaust pipe. The vehicle was taken to a local independent mechanic, where it was diagnosed, and determined that the transmission and catalytic converter needed to be replaced. The transmission was replaced. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 199,000.

The contact owns a 2013 Dodge Grand Caravan. The contact stated that the vehicle was taken to the local dealer, who replaced the motor mounts and transmission. Five months later, the transmission failed and needed to be replaced. The vehicle began driving roughly. The PCM update was completed by another local dealer, but the failure recurred. While driving at an undisclosed speed, the check engine warning light illuminated. The transmission was shifting through the gears erratically. The vehicle was not drivable. There were no warning lights illuminated. The coil packs and spark plugs were replaced at an independent mechanic. Several days later the RPM was fluctuating, and the vehicle lost motive power while driving up an incline. The vehicle was taken to an independent mechanic who determined that the failure was due to the torque converter failing. The local dealer was previously contacted, but no longer accepting calls. The vehicle was taken to the dealer, who stated there were no codes retrieved. The dealer informed the contact that there was no warranty coverage for the issue. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 163,000.

2013 Dodge Grand Caravan. Driver active headrest deployed without warning while sitting in garage and unoccupied. The loud noise and force of the defect could easily harm a passenger or cause a wreck. There are thousands of complaints on-line or through class action suits. I contacted the Chrysler Care group by phone on 10/22/2025. Their rep acknowledged, after asking if anyone was hurt, that this is a "known problem and a safety issue". After dealer inspection, Chrysler would provide a replacement even though it was after the extended warranty. The dealer performed the inspection on 10/24/2025. I notified the care group at this time. Weeks later the company has reneged on this commitment, refused to provide a replacement, and closed the case.

1 is the Tipm stopped working and dodge says the recall isn't for my VN# doesn't match recall , was lucky it didn't fail in traffic. 2. No recall on failed brake booster which could cause brake failure.

The vehicle randomly and dangerously stalls while driving at various speeds and locations. The vehicle will not restart until battery is disconnected and then reconnected. It then starts right up and runs fine until the next episode of stalling. This has been addressed by NHTSA campaign # 19V813 recall for bad fuel pump relay but my VIN # has not been included in this recall even though the vehicle is in the same year as the other vans covered under the recall. This is a very dangerous condition that keeps happening even after leaving the van with the Chrysler dealer for a week to diagnose and they say they don't have a definitive fix for it but offered to replace the entire TIPM for $1700 without a guarantee that the problem will be permanently fixed! The faulty fuel pump relay is well known and has been addressed with an external fuel pump relay kit covered by this recall. Please help us get our vehicle covered under this recall as we are retirees on a fixed income and have already spent too much money (close to $1000) trying to fix this known problem. Already replaced the ignition module which was another known possible cause but the problem still remains.

this is the fourth time i am filing a complaint. My 2013 dodge grand caravan completely dies and looses all power going over 50 miles per hour. no warning, no codes appear, nothing. looses all power, power steering, brakes, etc. have to get to side of road, wait 20 minutes, then car will start. this has happened now over 25 times. The latest time i was driving on the expressway and the vehicle behind me almost crashed into me. this continues to be a safety issue and it is unclear as to why dodge is doing nothing about this despite multiple complaints by hundreds of drivers. I have spent $3000 so far trying to fix this to no avail, new battery, fuel pump, fuel filter, TPM relay, new keys, new programming, still happens. case manager from 2023 told me to take to dodge dealer, spent additional $800, but they had no root cause. happened again. case manager told me to take to a different dodge dealer for second opinion. No codes, no root cause, but charged $400 diagnostic fee. When is Dodge going to do something about this? How many complaints are needed? does someone have to get injured? just tell me how to fix it, that is what i and thousands of others this happens to need to know before someone gets killed.

Passenger front seat headrest deployed while vehicle was parked. The plastic housing supporting a small metal bar broke causing it to open. I had no idea this could happen when I bought the car. I have kept this van longer than any I have owned as my Chrysler dealer closed and the van is no longer made! A new one form Chrysler is about $6-800! I was able to find a lightly used replacement headrest on Ebay and using a youtube video swapped it out myself. I have since learned Chrysler extended the warrantee 10 years, but my car is older than that. It only has about 65,500 miles on it. I am going to try and zip-tie the driver's side headrest so I do not get it in the back of the head when driving!! I presume the passenger side will remain intact for a while.

Driving down the road and my van cuts off. After leaving about 15 minutes it starts back up. I can go around 500 miles between cut offs. Had to 2 different garages with no solution. Replaced fuel pump, did not help. This is very dangerous especially on major highways.

Unknown what causes the issue. I've read it could be the tpim. I was driving down the highway when the insturment cluster stopped working, no speedometer, no gear indicator, gas gauge not working, no dash lights. the wipers turned on and would not turn off lose of both turn signals (I pulled over and checked) thankfully there was not traffic behind me when I was making a turn. Speedometer stopped working. No warning lights, insturments cluster was black. Not sure if my Brake lights were functional I couldn't check as I by myself. This has happened 3 times in the last month and a half. Not sure if the airbags are still functional. This is a major electrical issue.There are numerous forums of this same issue affecting different year/ models of Dodge and Chrysler vehicles.

While driving my van it will randomly shut off, it starts back up right away after I get to a safe location to do so but this is very dangerous. I've taken it to shops and they cannot replicate it and there are no codes on the reader to indicate any issues.

The contact stated that on several occasions while driving at various speeds, and the vehicle vibrated abnormally and then stalled. The vehicle was able to restart after several attempts. The contact stated that electrical tape was placed on the tip of the key fob to allow the extra space where the key fob and key fob ignition were not touching. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 140,000.

The contact’s son stated that the head restraint inadvertently imploded. No injuries were sustained, and no medical attention was needed. The dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and confirmed that the VIN was not included in NHTSA Campaign Number: 13V282000 (Electrical System, Air Bags) The contact was referred to the NHSTA Hotline for assistance. The approximate failure mileage was 121,887.

While driving our key in the ignition will loosen to the left and completely shut off all electrical for the car. The first time this occurred we were driving 55mph on the highway and had to safely steer off of three lanes of traffic with our kids in the backseat, at the time we thought it was a fluke. Now this is happening more frequently. It is the exact same issue listed under recall L25 “the ignition key inadvertently moves into the OFF or ACCESSORY position, the engine will turn off, which will then depower various key safety systems including but not limited to air bags, power steering, and power braking. Loss of functionality of these systems may increase the risk of crash and/or increase the risk of injury in the event of a crash.”

Active front seat headrests deployed without an accident for the second time. Had them replaced and paid for myself the first time years ago. Was compensated by FCA as part of special coverage after the fact. When they deployed again I brought it in and had it diagnosed at Hopkins dodge, it was covered under the special policy. They ordered parts. It took forever for the parts to come off back order. They just arrived in August 2024, I verified with their parts department. I am now told that I am out of the special coverage window and they won’t cover unless I pay the dealership over $200 to diagnose the same problem again. It has not changed. They already said it was covered and now they are trying to back out because it took forever for them to supply the required parts. I need this repaired like they said they would two years ago when I got it first diagnosed and the parts ordered.
