Despite the severe nature and force of this collision, the airbags failed to deploy fully. This malfunction directly compromised my safety during the crash.Vehicle Maintenance & Diagnostic StatusAuthorized Service History: All routine maintenance and vehicle servicing have been performed exclusively at the authorized dealership.No Prior Notice: The dealership has never informed or warned me about any potential defects, recalls, or issues regarding the airbag system.No Dashboard Warnings: Prior to and at the time of the accident, there were no warning lights or safety indicators illuminated on the dashboard.I request that this statement be formally added to my claim file and that a certified inspector thoroughly examines the vehicle to evaluate both the crash damage and this critical safety equipment failure.Sincerely,

UNKNOWN I was driving on an expressway around 2:00 AM June 22nd using my cars adaptive cruise control. There was light rain and my car kept a safe distance from the car in front of me that was going slower than the speed I had set my cruise control to. Suddenly my car accelerated as if the car in front of me was no longer there. I had to quickly apply the brakes to avoid a collision. I have around 8,000 miles on my car and I had no problems the last time I went on a trip using the adaptive cruise control.

The vehicle frequently displays a “Battery Charging” message on the dashboard whenever I come to a stop at traffic lights or stop signs. This message appears even when the vehicle is operating normally and there is no actual charging system problem. According to Toyota documentation and owner discussions, this message is triggered when the auto start/stop system decides not to shut off the engine due to the 12‑volt battery’s state of charge. However, the message is unnecessary, cannot be disabled, and provides no actionable information to the driver. It functions as a non‑critical status notification but is presented with the same prominence as a warning. This creates a driver distraction, as the message repeatedly draws attention away from the road for a condition that is not a fault, not a safety issue, and not something the driver can address. The message appears frequently during normal driving and cannot be suppressed. I believe this is a design flaw in Toyota’s start/stop implementation and dashboard messaging. The system should not display a warning‑style message for normal operating behavior, especially when it cannot be corrected by the driver. I am requesting that NHTSA log this issue as a potential driver‑distraction concern and encourage Toyota to revise the software to eliminate or suppress this unnecessary message.

Incredibly dangerous situation! Driving down I-15 in SLC when my Sunroof EXPLODED! 2024 Toyota Corolla Cross Hybrid, 22k miles, purchased new and sunroof came with the car. Sounded like a gunshot. Luckily, no one was injured by flying glass and kept the car stable. I could see how someone reacting to this sound might slam on their brakes or swerve the car or worse the driver could be cut by falling glass if the slider were open. My "damage" is having to pay for a new roof. Toyota dealer tried to say that something hit my car and it wasn't a manufacturer defect. Right. Tell me how a rock is going to get past my large Thule cargo box to drop a hole like this one! I filed a claim with State Farm but the part is special and must be ordered. UP until yesterday I have never had any issues with the sunroof. Such a racquet. Toyota gets to keep selling replacement sunroofs and the insurance companies and consumers are the ones paying for their poor workmanship.

Additionally, the vehicle was hesitating while accelerating and was losing automotive power. The vehicle was driven to the dealer, but the vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 150,082.

My 2024 toyota corolla xse hybrid had died 3 times because the 12 volt battery is no good. The battery has been replaced with the same type of 12 volt battery 2 times and the car still died. the car becomes inoperable. Hybrid cars, particularly Toyota, are experiencing significant 12-volt battery failures due to parasitic drain, leading to premature battery death. Affected models include the Toyota Corolla. The issue, often linked to faulty software, can cause the car to become completely inoperable. The vehicle’s multimedia screen intermittently displays a message prompting to “enter 6-digit PIN.” This message frame appears randomly and covers the center of the screen, including while the vehicle is in motion. When this occurs, the navigation display becomes blocked, creating a potential safety hazard as the driver’s view of navigation instructions is obstructed. To clear the message, the driver must manually press the “X” icon and then reselect the map display. This action requires driver attention and physical interaction with the screen while driving, increasing the risk of distraction and compromising safe vehicle operation. The issue occurs unpredictably—sometimes after several days, sometimes within minutes, or even immediately after pressing X to close the message. This recurring behavior significantly limits the use of navigation and other vehicle functions, effectively impairing normal and safe vehicle operation. Toyota dealer advised contacting the Brand Engagement Center. The Brand Engagement Center referred back to the dealer, suggesting the issue may be “mechanical.” The dealer reports that the vehicle is functioning normally. This creates a closed loop in which neither the dealer nor Brand Engagement Center accepts responsibility for resolving the issue. As a result, the safety concern remains unaddressed. Toyota should investigate and provide a definitive resolution to prevent the multimedia system from displaying the PIN prompt while the vehicle is in motion.

On July 2, 2025, while driving at approximately 8:35 PM my 2024 Toyota Corolla Cross suddenly and unexpectedly came to a complete stop. The vehicle's Pre-Collision System (PCS) activated without any obstacle, pedestrian, or vehicle in front of me. I was decelerating at about 15–20 mph in clear weather and road conditions. The abrupt stop was extremely dangerous and could have caused a serious rear-end collision had another car been behind me. This created a significant safety risk to me and to others. I had already completed the Toyota recall related to the pre-collision/braking system weeks prior. Despite this, the malfunction still occurred. The issue has not yet been confirmed or reproduced by a dealer, but I have contacted Toyota and scheduled a full inspection. The vehicle is available for inspection upon request. There were no warning lamps, messages, or error alerts before the incident. The car simply braked suddenly without warning. Out of fear for my safety, I have since disabled the PCS system. I am now afraid to drive the car, especially at higher speeds, and have requested that Toyota escalate the matter for further resolution or potential buyback.

January of 2025 the vehicle was put in drive and the vehicle shut down. "Power steering low egine stopped" message appeared. I Put the vehicle in park and started the car back up. I took it to the dealership. The battery was low they charged it and released it back. The vehicle was brought back shortly after that due to the vehicle being put in park and shutting down displaying the same "low power steering. Egine stopped" message. Along with "EPB stopped Incompletely". Brought it back in and the battery was low and they replaced it. A few months later the vehicle was brought in due to multiple braking events either when pulling forward with no acceleration (nothing was around the vehicle) or putting the vehicle into reverse with no acceleration (nothing behind the vehicle). During these events the car made a loud grinding sound and displayed BRAKE across the dash. The 3rd time at the dealership was due to the car shutting down when put into park and braking when reversing out of a parking spot. The code "epb stopped Incompletely" and the BRAKE message appeared.The dealership checked for codes but the car didn't have any saved. They can't recreate it. They had a feild tech come out but I'm unsure of what was found because they haven't released it. As of June 22, 2025 I was driving and the car braked on me while going 40 mph. The seat belt locked up and threw me and my passenger forward. A red BRAKE message appeared and the car sounded an alarm. The vehicle was brought back into the dealership on June 23, 2025 the dealership turned me away. Corporate started a case.

On [XXX], at the intersection of [XXX] and [XXX] in Los Angeles, [XXX] was operating the subject vehicle when it was involved in a hit-and-run that resulted in a secondary collision with multiple vehicles. Photos (attached) document the significant impact. Emergency responders were required to extricate [XXX]from the vehicle, during which they informed her that the airbags failed to deploy—a fact also documented in the official LAPD report. The failure of the airbag system represents a significant and dangerous malfunction of the vehicle’s safety features. This is particularly troubling in light of Toyota’s recent recall, which affects more than 400,000 vehicles. On January 10, 2025, I was driving my 2024 Toyota Corolla Cross and entered a parking lot to park. I started a three-point turn to park head-in and was slowly turning the steering wheel to the right. I was about to switch from the accelerator to the brake when the car suddenly accelerated rapidly. It felt like the front of the car lifted, and I lost control, colliding with a parked car to the left of the space. The distance between the two cars was about 5 feet, and the sudden acceleration lasted only 1-2 seconds. My car has a proximity warning and automatic emergency braking system, (Pre-Collision System) but neither activated, and the airbags did not deploy. My husband, in the passenger seat, heard a loud engine noise when the car surge forward unexpectedly. After the crash, I parked the car, checked for any objects near the pedals but found nothing. The other car had rear bumper damage, and mine had significant front-right damage, including a severely worn tire. I reported the incident to the police, exchanged information with the other driver, and called a tow truck. I later suspected a malfunction in the throttle or safety systems, such as the proximity warning or braking systems, which failed to activate. No prior warning lights or issues were present. The failure of these systems increased the risk to both myself and others in the parking lot. The cause of the malfunction has not been reproduced or confirmed by a dealer or service center. The vehicle’s onboard diagnostic (OBD) system could be checked for errors, as a potential cause of the sudden acceleration but was told that it is difficult to diagnose such issues and was told that it was slim to find anything even if I did a diagnostic check. Given the car’s advanced systems, I believe these components should be thoroughly inspected to prevent future incidents.

Brakes were malfunctioning and did not work. I was traveling South of Hwy 99 in the middle lane and noticed the vehicles in front of me were slowing down and braking. I attempted to applied the brakes but it was not working. I swerved to the left to avoid a collision with the vehicles in front of me. As I swerved to the left onto the fast lane, I hit the vehicle in that lane. Myself, along with the 3 passengers were transported to the hospital. The vehicle is towed and has not been inspected by the insurance company. Police was called to the scene. There are no recalls that I know of on brakes for my vehicle.

The headlights on this vehicle are not safe. They have a shadow , or curtain over the top half of beam. This severely curtails your ability to see safely down the road. Going up to a hill, your ability to see road hazards are almost nill. You cannot see hazards to the side of vehicle , or above, either. My wife is a school bus driver, and is aware you should be able to see 15 seconds in front of you, or a city block. This vehicle totally fails to illuminate anything near that. And you cannot read street signs, unless they have their own lighting. We took it to the dealer. They adjusted the lights, which didn’t solve the problem, and now we are getting flashed by other drivers, and are causing road rage. Please help!

Corolla Cross Hybrid SE- The tail lights are almost non existent. At night when I look from behind, the car is almost invisible from 50 feet away. Lic plate light is the only help. Looks very dangerous and scary to drive in the evening or in the dark. Please look into this.
