The contact stated that while driving approximately 55-60 MPH on a freeway, the blind spot monitoring system intermittently failed to detect vehicles in adjacent lanes. The contact stated that the failure nearly resulted in two crashes. There were no warning lights illuminated. The contact continued driving the vehicle to the residence. The vehicle was taken to a dealer within one week for inspection; however, the dealer was unable to duplicate the failure and no repair was performed. The contact stated that the failure recurred, and the vehicle was returned to the dealer, where it remained. The vehicle had not been diagnosed. The manufacturer was notified of the failure; however, no update was provided. The failure mileage was approximately 18,000.
The contact owns a 2024 Toyota Rav4. The contact stated that while driving approximately 30 MPH in a neighborhood, an advertisement unexpectedly appeared on the center display and obstructed the navigation screen. The contact stated that on the most recent failure, the navigation screen returned after a short while; however, on a previous occasion, the navigation screen could not be restored without restarting the vehicle. The contact expressed concern that the condition prevented access to the navigation system while driving. The vehicle was taken to a dealer; however, no cause for the failure was found. The vehicle was not repaired. The manufacturer was notified of the failure; however, no response was received. The failure mileage was approximately 17,500.
I was in a drive thru and my car had been idling for less than 5 minutes in the rain and started shuttering really bad with my roms dipping low. i put the vehicle in park and the the entire vehicle shut off and would not stay on without dying. for the next 20 minutes everytime i tried to start it it shut off immediately no matter what i would do. the vehicular kept telling me “Engine Stopped steering power low”. it let me put the vehicle in accessory mode and was fine but my steering wheel remained locked. after about 30 minutes i was able to get the vehicle to start and pull up about 15 feet before it shuttered and i put it in park and it died again. i had to get it towed to the dealership.
I purchased a 2024 Toyota RAV4 in March 2024 with the expectation that its advertised safety and driver-assistance systems would function as designed. The vehicle has experienced a failure of the forward camera/ADAS system, resulting in the loss of multiple Toyota Safety Sense features. The following systems are currently unavailable or malfunctioning: • Dynamic Radar Cruise Control / Adaptive Cruise Control • Pre-Collision System • Lane Tracing Assist / Lane Keeping Functions • Road Sign Assist (speed limit recognition) • Related driver-assistance and safety warning functions When cruise control is activated, the vehicle displays a warning that cruise control is unavailable. The dashboard reports multiple safety-system malfunctions. As a result, I have lost essentially all advanced driver-assistance features that were included with the vehicle. Toyota’s dealer diagnosed the failure and Toyota approved the repair under a goodwill case. However, I was informed that the required replacement component is on national backorder with an estimated wait of approximately two months and no guaranteed delivery date. Despite acknowledging the defect and approving repair, Toyota has not provided a comparable loaner vehicle or alternative transportation. I am being asked to continue operating the vehicle for an extended period without the safety technologies that were a significant factor in my purchase decision. I am concerned about the loss of collision-avoidance, lane-assistance, and adaptive-cruise functions, the indefinite repair delay, and the lack of transportation assistance while awaiting parts. I am requesting investigation, assistance, and consideration of repurchase or other appropriate remedies.

The trim around the sunroof is cracked and causing a leak. I took the vehicle to South Hills Toyota in Cannonsburg PA, for maintenance and state inspection, specifically identifying this issue. The car is less than 2 years old still under warranty Initially I was told it was not a material defect and not covered by warranty, remedy would be replacement of entire sunroof. Discussion with the service manager resulted in the defect being escalated to Toyota. Pictures were taken by the Toyota service team and sent to Toyota for further review.
I apologize if this does not meet the NHTSA bar of a safety hazard. It does reduce the ability for hands free navigation and acts as a distraction for drivers. Toyota Rav4s have very poor GPS tracking which makes the directions in the integrated head unit and guage cluster very distracting because they are consistently incorrect. There are dozens, maybe 100+, reports online of people having similar issues. Toyota dealership has looked at it many times, escalated for troubleshooting, and as of today they say there is no fix. I have many videos to demonstrate the issue is not a fluke. There are no warning signs and the GPS will just start giving poor directions. The "danger" is in the fact that the only workaround now is to engage with a phone or other device while driving. In theory a human can rise above this and be smarter than the car but isn't that true of many safety components like non-functioning wipers(drive slow) or blind spot protectiong(check your mirror). The car is giving bad directions that the user is relying on as a navigational aid.
2024 RAV4 XLE Premium will randomly pull to the left or right after start-up, or cold soak, and has been doing so since new. The Dealer has verified that the alignment is correct and there are no mechanical issues. When the vehicle is shut off and restarted, the symptoms will likely change and will often correct itself, but this sometimes changes the direction of the pull. This aligns with the same symptoms as NHTSA MC‑10214321 and Toyota Tech Tip T‑TT‑0616‑20. I have shared this information with the Dealer and they claim to be investigating the possible cause and I am waiting on confirmation of next steps to resolve it. Please update the known NHTSA complaint that has already been issued for Toyota RAV4 model years 2019-2021 to include later model years and including 2024.
After driving my 2024 RAV4 in rain for about an hour, I was then parked for about 5 minutes in a parking garage and the engine shut down and warning light came on for power steering, and white smoke /steam came out of the engine. Engine would not restart, even after tow truck arrived thirty minutes later. Once dealership received my vehicle they say it starts fine and can't find any issues. They have been unable to duplicate the issue. I saw this same issue online and have requested they check the MAF sensor and the battery load test. Seems to be a big issue in these cars.
While driving on the freeway for about 60 minutes, the drivers, driver side door that was locked released and opened up. The door was fully shut and no warning door ajar was lit up before driving. I grabbed the door and held it until I could exit and slow down to shut it. I tried to fully pull it shut while driving but was unsuccessful. I slowed down and had to pull the handle to release the lock and open the door more to have enough momentum to fully close the door again.

Driving on [XXX] merging onto [XXX] heading north, the sun roof exploded at approximately 70 mph. There were not vehicles near us at the time nor any obvious shrapnel or ricochet marks indicating an object struck the glass causing the break. The contact placed the key fob on the start button, and the vehicle started. While driving approximately 35 MPH, the vehicle stalled. The vehicle was pulled over to the side of the road. The contact pressed the start button, but the vehicle failed to restart. The contact stated that the message was displayed again, and the key fob was placed next to the start button, and the vehicle restarted. The dealer was contacted and advised to change the battery in the key fob. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 11,000.
I was driving down the Interstate and my sunroof exploded without apparent cause. This was in Colorado, and there was a change in altitude, so I thought perhaps a difference in pressure might have played a part. There were no cars in front of me nor any visible debris. I was just driving, and heard a loud bang over my head. My sunroof and shade were closed at the time.
Car is out of alignment resolutions in the rear passenger tire to wear down at an abnormal rate. Had the vehicle inspected by a local mechanic and two different dealerships. All three inspections came back with the same answer. The camber and the toe for the right rear tire can not be set to a center good alignment. Leaving it in the red zone that will result in excessive Tire wear, handling and safety problems.
My wife was driving down the freeway with the panoramic sunroof closed. The sunroof glass without warning made a loud popping noise and there was safety glass all over the interior of the car. She was not operating the sunroof at the time. The fabric sunshade was open at the time. Dealership refuses to cover the damage 2200 under warranty.

In addition, the contact stated that the failure caused her to crash into a rail guard, and the rear driver-side tire had deflated, and the rim was damaged. No injuries were sustained, and no medical attention was sought. No police report was filed. In addition, an independent mechanic arrived on scene to replace the tire. The local dealer was contacted; however, the vehicle was not diagnosed or repaired due to the warranty having expired. The manufacturer was made aware of the failure. The failure mileage was approximately 40,000.
My sunroof exploded while driving . Nothing hit my car there was no cars around me
After driving IN RAIN for about 27 Miles then idling in slow traffic, vehicle started running poorly, stalled and would not run properly. Got "Reduced Engine Power, Visit Your Dealer Message". Engine would start but stall when put in drive. I held up traffic for several minutes. After several minutes of restarting and stalling, I ran engine in park for 2000-3000 rpm for about a minute. The car then ran fine and I drove for over a week before I could get it to the dealer. It seemed to be run fine during this time. Dealer found code P010014, MAF sensor and replaced the sensor. Toyota TSB T-TT-0763-24 appears to describe this issue but it does not appear to have a real resolution for this. It is 'under investigation' Just replacing the sensor, if it is just another one of the same type does not give me confidence that this dangerous condition may not happen again. There are many reports like this one.
